Print the above forms, fill them out as completely as you can, and bring them to the:
Town Clerk's Office
Administration Building
149 Main Street
Watertown, MA 02472

These forms are not official until they are signed and certified in the Clerk's Office. Other required forms are available at the Clerk's Office.

What You Need

  • Application Form 
  • Annual Report Form 
  • Current Non-Profit STP-5 Form
  • First-time applicants must submit a statement of organization or bylaws

All completed forms must be returned to the Town Clerk’s Office for signature and issuance of permit. Cost is $10.00. The permit will expire one year from date of issuance.

  • One copy remains in Town Clerk’s Office
  • The clerk's office mails one copy to the state

  • One copy remains in Town Clerk’s Office
  • The clerk's office mails one copy to the state
